We offer Pacificare PPO health insurance
It is a 50/50 plan
$1000 deductible.
$3000 annual cap (covered at 100 percent after cap is met)
$35 copay for In-Network providers office visit
Perscriptions - $10 G/$35B/$50NF
no qualifying exams

Vision and Life Insurance available through Pacificare

Dental Insurance - United Healthcare HMO plan available

Monthly premiums differ for each employee
Canon pays $100 towards your premium monthly
Dependants can be added to your policy
Premium is deducted from payroll weekly
Coverage ends at the termination of assignment
Cobra is offered at that time

**Health Insurance and Vacation Days are not available to Independent Contractors

Available to all employees at no charge
Deposited weekly – Friday AM
